Durable Power of Attorney
In the State of California, Durable Power of Attorney allows an individual to make financial decisions in place of someone else. The Durable Power of Attorney grants these powers and decision making to a trusted friend or relative who will be known as an “Agent”. The Agent can only make decisions as specified by the “Principle”. |

Advanced Health Care Directive
In the State of California, the Advanced Health Care Directive allows a person to appoint a Health Care Attorney as well as to choose the types of care they do or do not want to receive. The person may allow his or her power as a health care lawyer to make medical decisions now or in the future if the person becomes disabled. In order to be accepted as a legal document, this form would require acknowledgment before a notary public or two suitable witness signatures. |
